The Knee Joints have elastic like soft tissue structures called Ligaments which provide flexibility to the joint. Two main types of ligaments in the knee joint are Anterior Cruciate Ligament (ACL) and Posterior Cruciate Ligament (PCL). People who are engaged in sporting activities can accidently injure these ligaments when the knee joint is twisted or gets a shock due to any kind of accident like fall or collision. The injury to the ligament can be a partial tear or a complete tear. When the ligament gets injured, the knee joint gets swollen and there is severe pain during movement. The first step to be taken immediately after the injury is to immobilize the knee joint and to apply ice pack on the joint. The cold pack helps in reducing inflammation in the joint and also helps in healing the injury. Movement should be done by using a support so that pressure is not put on the joint and the ligaments. The injured person should be immediately taken to an orthopaedic doctor for evaluation and proper diagnosis. The orthopaedic doctor would do a clinical examination followed by X-rays to see the extent of injury. In order to properly visualize the ligaments an MRI Scan of the Joint would be needed. The MRI scan can reveal the extent of injury as to whether there is any tear in the ligament and whether the tear is partial or complete.
Partial tear of the ACL or PCL can heal with time and with proper rest being given to the joint. Your orthopaedic doctor might give some medications to assist in faster healing and relieving pain. However if the tear is complete then arthroscopic repair may be the only option. In this type of surgery the orthopaedic doctor would make 3 small holes in the skin around the knee joint and through these holes the surgical instruments would enter inside the knee joint. The camera attached to the arthroscope sends high resolution images of the inside which are seen on a computer monitor and the broken ligament is attached together using an implant. If the ligament is broken from the point where it is attached to the bone then anchors are used to hook the ligament back to the bone. Sometimes the meniscal is also injured which is also repaired alongwith the ACL and PCL. After completing the repair the orthopaedic surgeon brings the surgical instruments out and closes the small holes using staplers. No stitches are required in arthroscopic surgery. The patient can be discharged from the hospital on the same day and can get back on feet using a walker. Weight bearing is avoided for the knee which has been operated for the next 10-15 days.
