Anal Fissure is a small cut in the lining of the rectum which causes discomfort and bleeding while passing stool. Haemorrhoids or Piles are swollen veins which protrude outside and also cause discomfort and bleeding while passing the stool. The Piles can be internal or external. The most common cause for both Anal Fissure and Haemorrhoids is chronic constipation resulting in hard stool. When you have to strain to pass the stool both the fissure and haemorrhoids can happen. There may be other causes for these conditions which include - straining during child birth, inflammatory bowel disease etc. One must approach a general surgeon for the treatment of these conditions as soon as possible. The diagnosis for both Anal fissure and the Piles can be quite easily done during clinical rectal examination. In some cases the surgeon might perform a simple procedure called Proctoscopy to clearly visualize the internal part of the rectum which is not visible to the naked eye. If the patient takes care of the digestive system and consumes fibre rich diet then the problem of constipation can be taken care of and early stage fissure or piles can be cured on their own.
Anal Fissures can be treated by diet modification and medication in most of the cases. In some cases when these things do not help then the surgeon can do a small surgery called Anal Fissurectomy in which a small incision is given in the sphincter to relax the muscle which allows the fissure to heal. Piles growth pops out of the rectum while passing the stool and then goes back inside. This is the early stage of piles and can be managed conservatively. If these methods do not help then the doctor can give you rubber band ligation in which a rubber band is placed around the piles growth which cuts the blood supply to it and thereby causes it to shrink. A chemical injection can also be given into the piles to make it shrink in size. This procedure is called Sclerotherapy. For advanced piles which are bleeding you would have to undergo piles surgery in which the surgeon either cuts the growth and puts stitches on the incision or uses a stapler to tie it into the rectum so that it does not trouble you anymore. The stapler piles surgery causes no blood loss and the recovery is also much faster.
